Grant Applications

Procedure for Receipt and Evaluation of Grant Requests

Application Guidelines

Organizations applying for grants should draft a proposal in accordance with these Guidelines for consideration by the Grants Review Committee. Each application should include the following:

(1) Proposal Summary: Each application should include a typed summary with provides the following information:

  • Organization Name
  • Chief Executive Officer
  • Brief Summary of the Project
  • Project Director
  • Address
  • Phone Number
  • Area of Interest
  • Mission Statement
  • Service Area

(2) Narrative Section: Each application should also include a Narrative Section. This section should be no longer than three (3) typed pages and should include the following information:

  • A detailed description of the project, including strategies, measurable objectives and a timetable.
  • A description of the challenge(s) to be addressed by the project.
  • A summary of the reasons for the project.
  • The amount requested from the Foundation.
  • A plan for continuing the project once the Foundation’s funding ends.
  • A plan for evaluating the project.
  • A concise history of the organization with an overview of current programs and activities.

(3) Attachments: Attachments should include the following information/documentation and should be no longer than fifteen (15) pages:

  • A copy of the Internal Revenue Service notice stating that the organization is tax-exempt, or a copy of the organization’s fiscal agent’s 501(c)(3) letter along with a letter from the organization stating its willingness to serve as fiscal agent.
  • A list of the organization’s board members including their principal occupations, a description of the terms of office and the rotation schedule for the board.
  • The names and qualifications of persons responsible for carrying out the program.
  • A detailed project budget and budget narrative including income sources and expenditures, as wall as a list of proposed funding sources.
  • Financial statements, including the organization’s operating budgets for the last completed
  • Supporting materials, such as reports, brochures and news articles.

The Grants Review Committee shall consider each grant application even if all of the above-described information/documentation is not submitted in conjunction with the application. However, the Grants Review Committee shall not recommend any grant to or on behalf of any organization unless the organization submits a copy of an Internal Revenue Service notice stating that the organization is tax-exempt.

Grant Request Deadlines

Grant requests should be received by 5:00 p.m. on the deadline date. If the deadline falls on a weekend, requests are due on the Friday which precedes the weekend. The deadline and notification dates are:

Reporting Requirement

If a grant is awarded, the Foundation requires that the funded organization submit both fiscal accounting and narrative reports on the use of its grant and the impact that the grant may have had on the community it serves. Reporting guidelines will be established in the letter of award.
